1. Provides rehabilitative and therapeutic services to patients in their homes or in the outpatient clinic.
a. Plans and executes rehabilitative treatment under the supervision of the clinical supervisor.
b. Conducts therapy sessions with patients in accordance with individual treatment plans, observing and recording patient response to therapy.
c. Designs adaptive, assistive and supportive exercises or movements for patients to improve motor function and strengthen and support muscles or deformities.
d. Evaluates the effectiveness of the current plan and patient progress and make necessary adjustments to the Plan of Care to ensure the best possible outcomes for the patients.
e. Completes patient visits in a timely manner and in accordance with agency policy.
f. Documents all services provided, observations, client progress, and recommendations according to established company policy.
g. Reports changes to the clinical supervisor in a timely manner.
h. Records patient progress data, keeps track of patient exercises and measures range of motion during therapy sessions.
i. Performs administrative duties such as answering patient inquiries and ordering supplies.
2. Participates in interdisciplinary activities per agency guidelines.
a. Lead client care conferences as requested to provide information to other members of the team.
b. Promotes patient referrals to other disciplines as appropriate.
c. Attends staff meetings and continuing education programs per agency guidelines to comply with personal professional growth needs and professional requirements.
d. Participates in the agency quality improvement activities pertinent to the role of Physical Therapist.
3. Promotes personal safety and a safe environment for clients and coworkers.
a. Maintains equipment, material and supplies to promote a safe therapeutic environment.
b. Recognizes and reports potentially unsafe situations.
c. Cleans and straightens patient treatment areas.
d. Demonstrates knowledge of safety and infection control practices by compliance with the agency policies and procedures.
4. Maintains confidentiality in all aspects of the job.
a. Respects confidentiality of information in the client clinical record and shares this information in accordance with agency policy.
b. Does not discuss client or staff identities, conditions, or circumstances in public or with others who are not professionally involved with the situation/person.
c. Protects written confidential documents in a manner that prevents unauthorized access.
5. Performs job in compliance with agency policies and procedures and professional and community standards.
a. Accepts responsibility in accordance with role of Physical Therapist.
b. Provides services in accordance with applicable state, federal, and JCAHO standards.
c. Attends meetings and required educational programs.
d. Accepts responsibility for personal and professional development and identifies
learning/developmental needs to supervisor.
